Appendix 1 – Helpful Tool for Organizing Patient Details or to Use During Rounds
Use this to organize the many details about the patient and healthcare team. This can be used to keep track of past events, present concerns, and future plans. Each prompt may not be needed, and other important issues can be added. Also, please write at least one option for “What am I doing for self-care today?”.
